PATIENTS' RESPONSIBILITES

Welcome to Northwest Center for Natural Medicine, we would like to bring to your attention a few things we expect that you will do.

• Call as soon as possible if you will not be able to keep an appointment. We require a 24 hour notice prior to your appointment.

• Bring your insurance card with you, insurance authorization if needed, and co-payment.

• If self pay, payment is due at the time of service.

• Verify with your insurance for eligibility.

• Be considerate of all personnel and patients.

• Pay for supplements at time of purchase.

• Supply us with complete and accurate medical history information.

• Tell us if there are changes if your health status.

• Tell us if there are changes to your insurance coverage or contact information.
